Endometrial ablation is an in-office procedure that destroys the tissue lining the uterus (the endometrium) to prevent chronic heavy menstrual bleeding caused by a very thick endometrium.
Ablation is usually performed in women who are through with childbearing and who do not wish to have a hysterectomy or who are unable to have a hysterectomy. Heavy menstrual bleeding is especially common among women who are entering menopause, but a thick uterine lining isn't the only cause of excessive bleeding. Before undergoing ablation, your doctor will perform a comprehensive exam to ensure other underlying conditions are not present.
There are several techniques, and your doctor will describe the technique they feel is best for you. Techniques include:
Once the lining is removed, scar tissue will form that reduces or stops menstrual bleeding. The procedure itself takes from 30 to 45 minutes to perform under sedation or local anesthesia. After the procedure is completed, you may experience some cramping or minor vaginal bleeding or discharge which can last for one to two weeks. Recovery is quick, and you can expect to go home shortly after the procedure is completed.
Nearly all women who undergo endometrial ablation will experience a significant decrease in menstrual flow, and about half will completely stop having periods. The procedure is especially effective in older women who are near menopause. Younger women may need to have the procedure repeated if heavy bleeding returns. Some younger patients may respond to a one- to a three-month course of hormones prior to the procedure to help thin the uterine lining.
